START

Introduction and Contents

START (Short Term Assessment and Reablement Team) / Sutton Health and Care is a multi-disciplinary team consisting of professionals from both Adult social services and the NHS, including Support Workers, Social Workers, Occupational Therapists and Physiotherapists.

These procedures are intended for use by Adult social service practitioners on the team.

The START service is provided free of charge for a time limited period, for no longer than six weeks and frequently for as little as one to two weeks depending on individual needs.

The aim of the service is to prevent hospital admission and to ensure the individual is able to be cared for in their home with appropriate care & support.

Throughout their START journey, the individual will receive a "hands-on" assessment, supporting or observing all aspects of daily living in the home. Equipment, technology, support and/or reablement techniques are recommended to maintain their independence.

At the end of the START journey the service will produce a detailed, person centred support plan to enable a formal assessment to take place.

Adult Social Care Internal referrals:

  • Hospital Pathway Team - A&E (prevention of admission) & Hospital discharge;
  • Locality Teams - Community new referrals & review of existing care plans / Safeguarding Teams - Outcome from a SVA investigation.
