A plan used primarily in mental health services to record the eligible needs that a person has, the Outcomes they want to achieve and the action that will be taken to support them to do so.
Care plans should be reviewed regularly to make sure that they are effective and to make any amendments or adjustments in a timely way.
In social care this kind of plan is known as a Care and Support Plan, or Support Plan.
