A plan that summarises all of the Outcomes a person wants to meet and all of the ways that they will be supported to do this.
A care and support plan should explain how a person intends to use any Personal Budget provided to them by the Local Authority, including any Direct Payment. It should also describe any other support they plan to access outside of this to achieve their outcomes. This could be through health services or within their local community.
People can write their own care and support plan or they can have help to do this from a family member, friend, social worker, advocate or any other appropriate person. Some local authorities commission a support planning service externally.
The Local Authority must agree the care and support plan and must review the care and support plan to ensure it is effective.
