Long Term Team and Move On Review and Support Team

Taking a multi-disciplinary approach, the Long Term Team work primarily with adults who have multiple and complex needs, and who consequently require on-going care and support over a longer period. They also work with adults who are difficult to engage and who may benefit from a more assertive outreach approach.

The Move On Review and Support Team works with adults to ensure that their accommodation continues to meet their care and support needs and support their recovery over time. Where accommodation is no longer appropriate they work with people to explore alternative arrangements and support them to move to somewhere more suitable.
