Life Planning (Transition Procedures)

An initiative of the London Borough of Barking and Dagenham, Life Planning is a co-produced model of practice that enables individuals and families to think ahead and make plans around their life goals, not just their immediate needs and risks.

Life Planning can be used to support disabled children, young people and adults but also parents and carers including young carers.

The Local Authority has developed practice guidance and a range of tools to support practitioners to understand Life Planning and have an effective Life Planning conversation. Click here to access it.
