Transition to Adult Care and Support

These procedures will support any social care practitioner to work effectively with a young person, young carer or parent carer through their transition to adult Care and Support.

They include procedures to carry out the following transition assessments under the Care Act:

  1. Child's Needs Assessment;
  2. Young Carer's Assessment; and
  3. Child's Carer's Assessment.

If needs are to be established under either the Children Act 1989 or the Children and Families Act 2014 you should refer to available children's procedures. (link to children's procedures).

If post transition needs are to be established you should refer to the section of this site that is relevant to your area of work or, if supporting a carer, to the carer's section.
