Learning Disability Team
The Learning Disability team work specifically with people over the age of 18 who have a Diagnosed Learning Disability. They carry out a full range of Statutory and Non-Statutory functions under the Care Act 2014, the Mental Health Act (sec 117 aftercare) and the Mental Capacity Act 2005 including assessment of needs and/or mental capacity, Care and Support Planning, review, safeguarding enquiries, best interest decision making and Court of Protection applications (for Deputyships or DoLS orders). The Learning Disability Team also joint works with NELFT health professionals and coordinates hospital discharges into bespoke community placements from Acute Psychiatric Assessment Units, Forensic settings and locked units.
