Hospital Activity Recovery Team
The Hospital Activity Recovery Team (HART) Team is part of an integrated Multi-Disciplinary Team known as the Integrated Discharge Team (IDT) (part of the Community Wellbeing and Independence Service). The Hospital Activity Recovery Team (HART) Team work will work with other hospital disciplines to manage safe, effective and timely discharges. The social workers will undertake Care Act assessments to determine the most appropriate discharge pathway for an individual, with Home First being the preferred/default option. They are able to arrange a range of services depending on the pathway, including reablement, equipment and where necessary longer term support including support for CHC funding/assessment.
