Monitoring and Reviewing Reablement (Community Activity Recovery Team)
1. The Role of Monitoring
Monitoring is a vital role in reablement and crucial in ensuring that:
- The Reablement Plan is working as intended;
- Progress is being made towards outcomes;
- Issues and risks to the plan are identified at an early stage; and
- The benefit of any equipment or assistive technology is being maximised.
2. The Purpose of Review
Review in reablement has a dual purpose to:
- Review the reablement provision itself; and
- Establish what the person's likely needs for Care and Support may (or may not) be when reablement ends.
The review conversation must broadly establish whether:
- The outcomes in the Reablement Plan have been achieved or not;
- The person has any new outcomes they want to meet; and
- Any improvements are required to the plan to achieve better outcomes;
- Whether there have been any changes to the person’s needs throughout the course of reablement; and
- Where there have been changes what those changes are, and whether or not the person is likely to need on-going support.
