Community Activity Recovery Team
The Community Activity Recovery Team (CART) is part of the Community Wellbeing and Independence Service. The team provide reablement and also carry out a range of assessments including Care Act assessment, Functional Independence Measures and Manual Handling assessment. Care and Support plans to maximise independence by facilitating access to community-led initiatives, arranging proportionate services or providing equipment, adaptations, direct support or therapy are developed and reviewed.
