Transition to Adult Care and Support
The Disability Enablement Service support disabled young people under the age of 18 to prepare for the transition to adult services. They also provide support to adults up to the age of 25 who have an on-going Education, Health and Care Plan (EHC Plan), either as the allocated team or through joint work with another team (for example the Learning Disability Team). They carry out a range of functions under the Care Act 2014 and the Mental Capacity Act 2005 including assessment of needs and/or mental capacity, Care and Support Planning, review, safeguarding enquiries, best interest decision making and Court of Protection applications.
