Learning Disability
The Specialist Learning Disability Service works with adults who have a Learning Disability. The team is responsible for carrying out the full range of functions under the Care Act 2014 and the Mental Capacity Act 2005, including assessment to establish needs, care and support planning, undertaking reviews and best interest decision making.
The Team also completes other specialist assessments and plans as required, working with individuals and their family/carers as well as with partner organisations and provide ongoing support to those people whose circumstances are more complex.
The Team staff a Duty Service for those individuals who do not have an allocated worker. The Duty Service can be contacted via LDDuty@Swindon.gov.uk or 01793 464820.
The Team consists of a Service Manager, Team and Assistant Team Manager working alongside Social Workers, Assistant Care Managers and Link Workers.
