Early Help and Help Team
The Early Help and Help Team is part of the Community Wellbeing and Independence Service. Team members carry out a range of functions, including the following:
- Screening all new Adult Social Care referrals and safeguarding concerns;
- Undertaking safety planning in response to safeguarding referrals that are BRAG-rated as urgent;
- Carrying out social care needs assessments in response to referrals that are BRAG-rated as requiring an urgent response;
- Transferring non-urgent (or specialist) referrals to appropriate teams elsewhere in Adult Social Care;
- Completing Wellbeing Plans with residents with emerging needs who are not yet eligible under the Care Act;
- Providing a range of information, advice, guidance and other short term intervention (including equipment) to maximise independence and promote wellbeing.
